Client Education Plan

1. Teaching is the process in which a person deliberately arranges a set of actions and elements that will be expose to a learner, and cause a change in his/ her behavior or the ability to perform new skills. Teaching can be affected by interpersonal communication, therefore the person must be prepare and had already assess the situation or predisposition of the learner (Potter, Perry, 2002 p474; Dirksen, Heitkemper, Lewis, 2004 p44).

Learning is the process by which a person acquires new knowledge that will help change his/her behavior or will develop new skills. There are three domains for learning the first one being, cognitive learning, which includes all the behaviors of intelligence; these behaviors are knowledge, comprehension, application, analysis, synthesis, and evaluation; the second domain is deals with the acceptance of feelings and opinions affective learning it like receiving, responding, valuing organizing, and characterizing; the last domain is psychomotor learning which involves the skills require for the integration of mental and muscular activity; these skills are perception, set, guided response, mechanism, complex overt response, adaptation, organization. Learning can be affected by the environment in which the learner is his /her motivation and the ability to learn (potter, Perry, 2002 p474; Dirksen, Heitkemper, Lewis, 2004 p44).

2. "In a patient's bill of rights, the American hospital association (1992) indicates

that, clients have the right to make informed decisions about their care" (Potter, Perry, 2002 p474). Therefore nurses have an ethical responsibility to teach their patients about their condition, care, and treatment. A nurse must carefully assess and evaluate their patient's need in order for education to be effective; an individualized plan should be done and implemented, and finally an evaluation should be done to identify if goals have been achieved (Potter, Perry, 2002 p474; Dirksen, Heitkemper, Lewis, 2004 p44).

3. Hypertension

Hypertension is defined as a constant systolic blood pressure (SBP) greater or equal to 140mm Hg or a diastolic blood pressure (DBP) greater or equal to 90 mm Hg. There are two types of hypertension, primary hypertension, which is, elevated blood pressure without an identifiable cause, and secondary hypertension which is a result from other medical problems (Dirksen, Heitkemper, Lewis, 2004 p780).

The causes of hypertension are unknown, but several factors that contribute to hypertension are; over production of sodium-retaining hormones and vasoconstrictors, an increase in sodium intake, more than body requirements, diabetes mellitus, excessive alcohol intake, and an increase in sympathetic nervous system activity. Hypertension causes the heat to work harder than normal; it also puts strain in the blood vessels throughout the body, there for having complication in body tissues and organs; the most common organ disease are hypertensive heart decease, cardiovascular disease, peripheral vascular disease, nephrosclerosis, and retinal damage (Dirksen, Heitkemper, Lewis, 2004 p780).

Treatment for hypertension includes an adequate diet, weight loss, restricted sodium intake, limitations in alcohol consumption, physical activity, limitations on tobacco use; these modifications are used for 1 year before any medications are used. The medications for hypertension include diuretics, beta blockers, Ace inhibitors, and calcium channel blockers. (Dirksen, Heitkemper, Lewis, 2004 p784).

4. Cognitive learning Ð'- involves Knowledge, comprehension, application, analysis, synthesis, and evaluation. Bob needs to acquire knowledge about hypertension what the risks are and the potential complications; he needs to comprehend how changes in his life style will improve his condition. He needs to apply all the information receive to incorporated in a plan; and synthesized the possible manifestations
